Daybell&Choo Terms & Conditions
Eyecare Plan T&Cs
Thank you for agreeing to join the Daybell & Choo Eyecare Plan and agreeing to abide by these terms. Your membership benefits start when you sign up, and subject to you making Direct Debit payments on time, this will continue until terminated as set out below.
Membership of Eyecare Plan entitles you to:
● Enhanced eye exams including OCT and Optomap® tests when required by your eyecare professional.
● Priority appointments.
● Discounts on eyewear.
● 18 month accidental damage warranty on new eyewear purchases.
● Hearing assessment.
● A contact lens fitting.
Payments:
● Payments of your membership fees shall be by way of direct debit.
● All direct debit payments will be made to Daybell & Choo via Eyecare Plan Limited and are protected by the Direct Debit Guarantee.
● In the event of the Direct Debit not being paid within 7 days after the due date, we reserve the right to suspend the provision of products and services.
● If your level of eyecare requirements, products ordered or product price change, we shall need to change your Direct Debit to reflect those changes. However, we shall seek to give you at least one months’ notice where we need to do so.
Data:
● We shall handle any personal data that you may provide to us under this Agreement in accordance with the Data Protection Act 2018 and the General Data Protection Regulation (UK GDPR).
Spectacle Discounts:
● Available only on new complete pairs of glasses (excludes reglazes).
● Discounts are applicable upon sign-up (subject to minimum term agreement).
● Only the individual named on the plan is eligible for discounts on glasses purchased for their own use.
Accidental Damage Warranty:
● Does not cover fair wear and tear.
● Loss and/or theft are not covered.
● Available only on new purchases made while a member of the Eyecare Plan.
● Each eligible purchase is covered by the 18 month warranty starting from the date of order.
● The 18 month warranty does not reset after a claim is made. For example, if a frame breaks after one year and is replaced under warranty, the new frame will continue with the remaining warranty period from the original purchase.
● Reglazes are not covered.
Eyecare Plan
● A 12-month minimum term agreement is required. If the plan is cancelled before this term is completed, outstanding charges may apply.
● Monthly direct debit payments will continue until cancelled by the plan holder.
● A 1-month cancellation notice is required.
Contact Lens Fitting
● The Eyecare Plan includes one contact lens fitting.
● The included fitting excludes all specialist and Ortho-K lenses.
Hearing Care:
● Excludes wax removal appointments
Miscellaneous:
● This Agreement is exclusive to you and is not transferable to family members.
● You must be over the age of 18 to enter this Agreement.
● If the Eyecare Plan is cancelled during the 14-day cooling off period, you will be required to pay back the costs of any appointments and discounts received.
● Failure to comply with the T&C’s may result in future ineligibility to enroll in the Eyecare Plan
● No variations of this Agreement shall be effective unless agreed in writing by us.
● This agreement constitutes the entire agreement between the parties and supersedes and extinguishes all previous agreements, promises, assurances, warranties, representations, and understandings between them, whether written or oral, relating to its subject matter.
